PDF output revisited

There has been some concerns about the not-working PDF output
mechanism... It *does* work (see attachment) with the following tools:

DocBook XML DTD: 4.1.2
DocBook DSSSL stylesheets: 1.62
Xalan-C: 1.0
LogReport: CVS version 2001-03-21

On Debian Linux at least...

So please send me version combo's which do not work and please include
the (anomized) logfile from which it should make a report and the service and
superservice used.
